Intake Form –
The intake form helps our organization track who we care for and how we are doing. The information collected helps our organization see the needs to fill in the community and apply for funding opportunities to extend our reach to those communities. If you did not receive this form, it may be found using the button provided.

General Liability Waiver –
The general liability waiver can be found using the button provided. Please download the form, print it, sign it, and bring it with you to your scheduled session. If you don’t want to complete the form here, you can go ahead and complete one at the ranch. Family units only need to complete one form for all members. Authorized signers must be 18 years or older.

If you don’t want to complete the form here, you can go ahead and complete one at the ranch. Family units only need to complete one form for all members. Authorized signers must be 18 years or older. Note: This form is for special events only. (i.e., trail rides, clinics, etc.) This form is intended to protect the ranch, its guests, and the featured presenter during the event.

Safety and Health Considerations –
The safety and health considerations form helps us to individualize your experience at the ranch. We wish to ensure that you are safe so you can focus on the reason you came to the ranch in the first place. This form is optional but recommended. The form can be found using the button provided.

Non-Disclosure Agreement –
The non-disclosure agreement can be found using the button provided. This form will remain optional but available. We respect the privacy of our members and the sensitive nature of conversations that may happen during sessions. If you wish to use this form, please download the form, print it, sign it, and bring it with you to the ranch. Or one may be provided at the ranch upon request. Note: G2G will follow the outline of this form in regard to reporting as needed to ensure safety for everyone.
Non-Disclosure Agreement
G2G Safety Plan –
The G2G safety plan is an optional document that is for personal use. This form is part of what makes us different. At G2G, we believe that it is better to have a plan for when things go wrong than to try to figure things out afterwards. This form can be downloaded, completed, and kept somewhere safe so your support can reference it as needed. Note: This form is not meant to diagnose or treat medical conditions.

Continuum of Care –
The continuum of care is an optional program that we hope fills a gap in the care of first responders, veterans, and their families. We noticed that after a member is given resources, they are expected to figure out the rest of the journey on their own. This is wrong. We also noticed that after the initial event, follow-up from organizations, resources, departments, and sometimes friends falls off. This is also wrong. People don’t decide how or when something will affect them, but we want to be there and support you when or if it does. This form will allow you to choose three dates during the year that are meaningful to you. Good or Bad! You can tell us why that date is meaningful, if you want us to contact you, and how you want to be contacted (call, text, email, etc.).
